The OpenCrypt Surveys and Input Forms add-on offers an easy to use system for managing all your online surveys, questionaires and data input forms with one centalised and highly customisable system.
 
The system offers support for unlimited surveys/forms, all of which can consist of an unlimited number of pages, all of which can include as many input fields/questions as you required.
 
Each input field or question is highly customisable, allowing you to specify text, textarea, checkbox, radio and drop-down menu input fields. All fields can have preset values/options and can be optional or required.
 
Each page can include a customised header and body text, as can the welcome and thank you pages for the form. Each survey/form can also have an automatic end date so the form is no longer accessible after a set period of time.
 
Comprehensive administration and management tools are included for creating and managing forms and for viewing results. Forms are automatically generated and all you need do is link to them.
